At the conclusion of your child's hearing test, Kidsonic will email you a report. Most kid's will receive an "all clear" report, however around 20% of kids' tests will produce a result requiring a follow up appointment or referral. 

After submitting the form, please be patient as payment page will load for you to complete payment online for $42.50. A payment receipt will be emailed to you.
